The Federal Budgetary Healthcare Institution "Privolzhsky District Medical Center" of the Federal Medical and Biological Agency (PDMC FMBA of Russia) is a multidisciplinary federal medical institution established by Order No. 375 of the Ministry of Health of the Russian Federation, dated October 17, 2001.[2] It is located in Nizhny Novgorod at Nizhne-Volzhskaya Embankment, 2. The Center is one of the leading multidisciplinary clinics in the Volga Federal District and the only institution in the Nizhny Novgorod Region that regularly performs kidney and liver transplantation from both living and deceased donors.[1]

Structure and Scope of Operations

The PDMC comprises four clinical hospitals, five outpatient clinics, and a consultative and diagnostic clinic in Nizhny Novgorod, a Transplantation and Organ Donation Center, a mobile hospital, and three branches in the Nizhny Novgorod and Yaroslavl Regions — including a hospital in Borsky District and facilities in Rybinsk and Yaroslavl.[1][2] The attached population numbers approximately 60,000 people: the center serves employees of several dozen organizations in accordance with Russian Government Directive No. 1156-r of August 21, 2006.[1]

Outpatient care is provided across ten structural units; more than 2,000 people receive medical assistance per shift. The bed capacity allows for the annual admission of more than 18,000 inpatients. The PDMC employs approximately 2,000 staff members, including around 400 physicians covering virtually all specialties. Approximately 13,000 operations are performed annually across 14 clinical profiles, of which more than 4,500 fall under high-technology medical care (HTMC).[1] In terms of HTMC quotas, the PDMC ranks first among medical organizations in the Nizhny Novgorod Region and fifth within the FMBA of Russia system.[4]

Transplantology

The organ transplantation program was launched in March 2006 with a living-related kidney transplant. In 2007, the Privolzhsky Coordination Center for Organ and Tissue Donation was established at the PDMC; in May 2009, a deceased-donor program was launched jointly with the Ministry of Health of the Nizhny Novgorod Region. In 2008, the first living-related liver transplant was performed, and in 2009 — the first deceased-donor liver transplant; from September of the same year, the center has operated a surgical organ transplantation unit.[1]

On October 25, 2016, the PDMC performed the first successful pancreas transplant from a deceased donor in the Volga Federal District; the center joined the select group of three institutions in Russia where this operation was performed for the first time. The unit has introduced laparoscopic organ retrieval technology. In total, more than 500 kidney, liver, and pancreas transplants have been performed since 2006. Two female patients with transplanted livers have given birth to healthy children. In 2020, the center's donor teams facilitated the first heart transplant in the Nizhny Novgorod Region.[1][4]

Oncology and Interventional Surgery

More than 1,200 HTMC quotas are fulfilled annually at the PDMC for the surgical treatment of oncological conditions of various localizations: the female and male reproductive systems, the gastrointestinal tract, the breast, the adrenal glands, the central and peripheral nervous systems, the thyroid gland, and soft tissue and retroperitoneal tumors. The center practices a closed-cycle treatment approach for advanced metastatic colorectal cancer and pancreatic cancer, performing more than 150 anatomical liver resections per year, which places it among Russia's leaders in this field.[1]

The department of X-ray surgical diagnostic and treatment methods performs more than 400 chemoembolization procedures for focal liver lesions and approximately 400 bile duct interventions annually, placing the center second in Russia in this field.[1]

Coordination, Education, and Achievements

Since August 2020, POMC has been designated the coordinator of activities of FMBA of Russia medical organizations in the Volga Federal District and manages patient routing within the three-tier structure of the district.[4] The center serves as a clinical training base for nineteen departments across four educational institutions that train physicians.[4]

POMC became the first institution in the FMBA of Russia system and in Nizhny Novgorod Region to receive certification from the National Quality Institute of Roszdravnadzor. Clinical Hospital No. 3 of the center is the first inpatient facility in the country to operate on the principles of lean healthcare; this result was achieved in collaboration with state corporation "Rosatom." During the COVID-19 pandemic, an infectious disease hospital was established on the basis of Clinical Hospital No. 2 to contain outbreaks; two physicians of the hospital were awarded the Order of Pirogov by decree of the President of the Russian Federation.[4]

Since 2003, POMC has been directed by Sergey Vladimirovich Romanov (born 1967) — Doctor of Medical Sciences, associate professor, and graduate of the Kirov Gorky State Medical Institute. By Decree of the President of the Russian Federation No. 274 of June 13, 2019, he was awarded the honorary title "Honored Health Worker of the Russian Federation."[3]
