Pyotr Nikolayevich Cheremukhin (born September 11, 1986) is a top-category Nizhny Novgorod neurosurgeon and deputy head of the neurosurgery department at the N. A. Semashko Nizhny Novgorod Regional Clinical Hospital. He is known for performing, in September 2026, the first operations in Nizhny Novgorod to decompress the vertebral arteries in patients with Kimmerle's anomaly.[1]

Education and workplace

Cheremukhin graduated from the general medicine faculty of the Nizhny Novgorod State Medical Academy (NizhGMA) in 2009 and completed his residency there as well.[3][4] He works as a neurosurgeon at the N. A. Semashko Regional Clinical Hospital (190 Rodionov Street), where he serves as deputy head of the neurosurgery department.[4][5]

Operations for Kimmerle's anomaly

Kimmerle's anomaly is a condition in which additional bony structures (bone bridges) compress the vertebral arteries at the point where they pass toward the base of the skull, impairing the blood supply to the brain and raising the risk of ischemic stroke in the vertebrobasilar territory. The anomaly may be congenital or acquired.[1]

In September 2026, Cheremukhin performed the first operations in Nizhny Novgorod to decompress the vertebral arteries in this condition. The procedure is performed under a high-magnification operating microscope: the surgeon isolates the vessels and removes the bony structures pressing on them. During the operation, the microscope is set to its maximum 25x magnification, and the bone bridge is cut away with a neurosurgical drill using microinstruments.[6] Cheremukhin himself described such operations as "practically jeweler's work": the surgeon works directly on the major vessels of the brain, and the slightest damage to a vertebral artery threatens severe bleeding or stroke; the procedure demands microvascular surgery skills and great precision.[5]

One of the first patients was a young woman born in 2006, who since childhood had suffered from headaches, unstable blood pressure, dizziness when turning her head, and episodes of loss of consciousness. Examination revealed bilateral compression of the vertebral arteries by bone bridges. Cheremukhin removed the bony formations on both sides; a week after the operation the patient was discharged, and at a follow-up appointment she reported that the headaches and dizziness had stopped. The first patient, a 19-year-old woman, was completely free of headaches a month after discharge.[6][1][5] The second patient described was a 60-year-old man with unilateral compression of a vertebral artery; after the operation his symptoms also disappeared.[2] By the time the reports were published, Cheremukhin had performed four such procedures, and several more patients were undergoing examination and preparation for surgery.[5]

The doctor recommends that people with chronic headaches and dizziness, especially when it occurs while turning the head, have the condition of the blood vessels of the neck and brain checked with CT angiography, which can reveal both vascular abnormalities and the bone structures compressing them.[1][5] The introduction of new diagnostic and treatment methods is taking place as part of the national project "Long and Active Life."[5]
